Communication in Groups and Organizations

I was thinking about our class discussion from yesterday and how the issues could have been resolved. It seems like the only way to fix the situation is to prevent it from happening in the first place. A lot of the solutions that we discussed involved things that the group needed to do at the start of the project. For example, one solution we discussed was that they needed to clearly define roles, tasks and a leader at the beginning of the project. So what could the team have done halfway through the project to fix the problems that it had? I think they could have tried to make each group member's work more visible (maybe during a final presentation or something). But is there anything else they could have done?
