Communication in Groups and Organizations

When I first saw that we had to read about an accidental military helicopter shooting, I wondered how it would relate to any of the material we learned this semester. But after doing the reading and sitting through class Tuesday, it definitely seems like this story directly relates to a lot of the concepts that we've learned. There was a complete breakdown of communication in a number of areas which eventually led to the accidental shooting. It seems hard to place any blame on any one group because so many people messed up. I know we are going to have a discussion on who's to blame tomorrow but I was wondering what everyone thought? It seems like each of the individuals in the air made at least one mistake so that leads me to think that the bigger problem was the overarching organizations/people above those in the air during the shooting.
